PHP form to email

Does anyone have an easy to implement yet flexible PHP form to email processing script.

Ideally I'd like it to be able to take on the exisiting design of my site or be able to redirect to a thanks for your feedback page once it has done its business (so to speak).

PS - I'm no PHP wiz, so please use plain English when explaining it.
