Right then, Windows XP, the little new folder button at the top of the save/open dialog box doesn't work, it just does nothing meaning I keep saving files with the name I want to call the folder.

Dunno how it broke or when I'm afraid

Can you not right-click in the window itself and create a new folder that way as a workround?

Yeah that works fine but I like my little button.

It seems to have magically started working again though, I'll see if it stays working and then just not ask why if it has indeed fixed itself.
