Someone at Five News called Gay Flashman (I think it was Five News)
There's a manager type bloke at our work called Dick Mean. Any emails we get at work have their surname first although as a special cause for him, his name appears as Richard Mean at (company) Bit gutting really as I waited a year to get something from him, just to see if he does have his surname appear first and I was gutted with the outcome!
Also with a name thing, I work for a cheque clearing company. Oh the glamour! and was quite interested today to pick up a cheque whose company name was "Fuchs Lubricants Ltd" Pure class, albeit sad but raised a smile anyway.

One of the lecturers at college had the name of Malcolm Dick.
He was also the head of the History Department at the college before being made redundant.
Therefore, "Malcolm Dick Head of History".
